The rule in one sentence
A Central Sterile Supply Department is designed around a single principle: instruments move in one direction only, from dirtiest to cleanest, and never back. Every wall, pass-through hatch, and staff routing decision in a CSSD floor plan exists to enforce that rule.
The three zones are consistent across almost every facility, regardless of size:
- Decontamination — where used instruments arrive, get sorted, and go through manual pre-rinse, ultrasonic cleaning, and washer-disinfector cycles.
- Clean / packaging — where inspected, dry instruments are assembled into sets and wrapped or pouched.
- Sterile storage and issue — where processed loads cool, get logged, and wait to be dispensed back to point of use.
Why “one-way” is stricter than it sounds
This isn’t just a suggestion about which door to use. In a properly designed CSSD, decontamination and clean/packaging are physically separate rooms with their own air handling — decontamination typically runs under negative pressure relative to the clean side, so airborne contaminants and aerosols from washing don’t drift into the packaging area. A used instrument, or a staff member who just handled one, does not pass back through a clean zone to reach a dirty one.
Where floor space forces zones into a single room, the fallback is procedural separation: dedicated PPE per zone, a strict order of tasks, and physical barriers like a pass-through washer-disinfector that loads from the dirty side and unloads from the clean side. It’s a compromise, not the design goal.
Where the sterilizer sits
The steam autoclave sits at the boundary between clean/packaging and sterile storage — loaded from the clean side, unloaded from the sterile side, ideally through a double-door or pass-through unit so the two zones never open into the same space at once. Even single-door tabletop autoclaves common in smaller clinics are placed so the loading bench is clearly on the clean side and the unloading bench feeds directly into covered storage, not back through the packaging area.
What breaks the model — and what to check for
The most common real-world violations aren’t dramatic. They’re small: a staff member carrying a phone or a pen between zones, a cart parked so it blocks the one-way path and gets used as a shortcut, or trays cooling on the same counter used for wrapping. None of these show up on a compliance checklist until they cause a traceable contamination event — which is exactly why zoning audits focus on foot traffic patterns and pinch points, not just room labels on a floor plan.
